Enrollment
ALL STUDENTS MUST HAVE THE FOLLOWING INFORMATION IN ORDER
TO REGISTER AT LANIER HIGH SCHOOL:
PROOF OF RESIDENCY The following forms of residency will be accepted for enrollment: 1) a copy of the deed/contract/lease agreement to your current residence, or 2) a copy of a current utility bill. (Proof of residency must show current address along with parent or legal guardian’s name. If you do not currently own, rent, or lease your place of residence, a notarized Residency Affidavit is also required). This form is available in the guidance office.
WITHDRAWAL FORMS FROM PREVIOUS SCHOOL
TRANSCRIPT OR REPORT CARD FROM PREVIOUS SCHOOL.
ATTENDANCE RECORDS- This is not a requirement for enrollment but recommended.
DISCIPLINE RECORDS. Discipline records from the previous school or a statement from the previous school stating the following: Student has no discipline record.
HEALTH RECORDS includes the following: 1) Georgia Immunization (shot records), and 2) Dental, Vision, and Hearing Certification. (Both records MUST be on Georgia forms. These forms may be obtained from the Gwinnett County Health Department or your own physician).

INDIVIDUAL EDUCATION PLAN (IEP). Required only if your child receives services through special education.
SOCIAL SECURITY NUMBER. Your child’s social security number is part of their permanent educational record, and it is required to receive the HOPE Scholarship. (Please provide this information on the Enrollment Form.)
STUDENT MUST BE ACCOMPANIED BY A PARENT OR LEGAL GUARDIAN.
TEMPORARY GUARDIANSHIP. This guardianship must be granted from Gwinnett County Juvenile Court, and a copy provided at the time of registration, in order to enroll a student by anyone other than the natural parent. (This should be done two weeks prior to registration).
STATE LAW
Students in the sixth grade or higher transferring to a new school will be required to provide academic and disciplinary transcripts to school officials. Students will be required to disclose conviction of designated felony acts and whether they currently are serving a suspension or expulsion from another school, the reason for such discipline, and the term of the discipline. Schools can refuse to admit students under disciplinary action from other school systems.
In lieu of compliance, a student may be admitted on a conditional basis if he/she and his/ her parents/ legal guardian executes a document providing the name and address of the school last attended and authorizing the release of all academic and disciplinary records to the school administration
